Ana Favareto, DNP, FNP-BC, APRN is a board certified Family Nurse Practitioner with over 10 years of experience in the medical field, she received her doctorate degree from Florida Atlantic University in Boca Raton, Florida. She is passionate about helping others heal, as well as creating a community of people who can support each other on their health and wellness journeys. She believes that the foundation of healthcare is connection, and highly values the time spent listening to her patients and hearing their stories. Her holistic approach to medicine helps her address her patients’ needs on a deeper level and to guide them in their journey to wellness.

Carly McMinn, MSN, RN is a masters prepared nurse with a background in emergency nursing, IV nutrition, and health coaching. After going through a season of healing that involved rebuilding a relationship with her own body, she developed a deep passion to connect others with their own physical wellbeing. She is not only an expert in her field of IV nutrition, but also an impactful health coach and powerful ally to her patients on their wellness journeys. Her intuitive perspective and empowering approach takes patients to a deeper level of healing.

Dr. James Lim is a board-certified emergency medicine physician and a graduate of the State University of New York at Buffalo and Albert Einstein College of Medicine in Bronx, New York.  He has spent the last ten years of his career caring for many acutely ill patients on the worst days of their lives. Through his experience, he has seen a tremendous need to develop connection and accountability with his patients to assist them in achieving their health goals. He believes not only in caring for patients who are sick, but also creating a community of support to help patients cultivate long-term health

What if I need medical attention when I am out of town?

As a member of Juniper Health you have access to our team through your membership at any point, regardless of your location!

How will I reach my provider?

You can call, text message or schedule a telemedicine appointment at any point.

Will I pay a co-pay for office visits?

No co-pays, just your monthly membership fees.  

Do you accept members with existing medical problems?

Of course! Your state of health should not affect your ability to obtain healthcare!

What about laboratories and other tests?

Labs, imaging and other procedural tests are negotiated with providers and the prices are passed directly onto you.  You will pay what we negotiate it to, which you’ll find is a large discount off what is charged to insurances.  

Are there hidden fees?

No hidden fees.  Anything not included in your membership fees will be detailed out to you.

Do I need an appointment?

We ask that you schedule with us for any telemedicine visit.  We are always available for texts and calls, and we will get back to you as soon as available.  Please allow some time for replies, especially during off-hours, but do not hesitate to reach out.

What if I have Medicare?

Unfortunately, due to federal regulations, at this time we are unable to see Medicare patients.  We are working on changing this in the near future!

What about the management of chronic pain? Do you prescribe narcotics?

We are not a pain management clinic, however, our purpose is to bring wellness and improve the lives of all of our members and our community.  If there’s any question or concern, please don’t hesitate to schedule a free call with one of our providers to see if we can help you!
